RESUMO

The objective of the study is to determine the possibility and the right to choose the directions of self-education and self-development by forensic medical experts within the framework of personal and professional development program in the educational trajectory formation. The materials of special and encyclopaedic literature including pedagogical and sociological sciences, as well as legal framework regulating the processes of continuing medical education have been studied. The choice of educational trajectory of forensic physician depending on personal benchmarks, self-education building and participation in educational activities has been defined.

RESUMO

The paper provides a rationale for a procedure to mold diagnostic competences in medical workers of the laboratories of therapeutic-and-prophylactic institutions and hygiene and epidemiology centers, Russian Federal Service for Supervision of Consumer Rights Protection and Human Welfare. The methodical features of molding diagnostic competences in the above contingents are the design and organization of an educational process by applying systems integration and competence-based approaches; increased active self-directed learning of audience; a procedure to organize its unsupervised extracurricular activities. Professional habits and skills in laboratory specialists should be molded on the basis of didactic principles and in compliance with the found methodical patterns. The eventual result (molded competences) and its compliance with the practical health care requirements is assessed using all control types (incoming, running, intermediate, and ultimate ones). This ensures the stability and predictability of molding diagnostic competences in parasitology specialists.

RESUMO

In the context of theoretical analysis, the paper characterizes a specialist training system based on the biological component of the qualification characteristics and professional standard of preventive medicine specialists. Evidence is provided for the relationship between the components of the training system for preventive medicine specialists. The authors also specify the fact that the content of qualification characteristics and professional standards, besides general requirements, must take into account requirements for training the specialists in terms of natural and climatic and socioeconomic characteristics of regions.

RESUMO

The intensive modern Russian reforms in professional education require that stafftraining in medicine should be also improved. The main directions and ways of improving education in the specialty of Parasitology are as follows:--to update professional education, by applying a competence approach, in terms of which the main focus is on mastering the activity and obtaining the experience in make this activity in different situations;--to apply the modular principle in the design of educational programs and a teaching process;--to improve the fundamental training of staff in the area ofparasitology;--to enhance the integration of special, related, and basic disciplines in stafftraining.

RESUMO

By using the training of medical parasitologists in malaria control, the authors unveil methods for teaching medical parasitology. In the context of a module approach in the additional professional education of physicians, they show how to arrange a teaching process whose goal is to provide a set of the professional competencies of trainees as a means of its achievement--the module construction of the content and structure of professional education.

RESUMO

The paper describes the development of alopecia with a patient with echinocccosis alveolaris during treatment with Nemozole (albandazole). To decide to continue or to discontinue Nemozole treatment in the development of alopecia, the patient should be given full information on the risk of alopecia to his life and quality of life as compared to the sequels of recurrent hydatid disease when Nemozole is discontinued.

In November 2000, an outbreak of Giardiasis intestinalis with aqueous transmission was recorded in one of the microdistricts of Perm. Thirty six children aged 1-7 years fell ill, 2 parasitic carriers were identified among the staff. The diagnosis was parasitologically verified in all children with clinical manifestations. Enzyme immunoassay was positive in 87.5% of the patients with the parasitologically verified diagnosis. The aqueous nature of the outbreak was confirmed by the detection of Lamblia cysts in the samples of water from the transfer network of the microdistrict in November 2000 and by the higher incidence of giardiasis among the microdistrict's residents since April 2000. Macmiror in age-specific doses was used to treat patients. The therapeutical efficiency of one course was 100%. If outbreaks of intestinal infections of unknown etiology occur, a parasitological survey should be made among patients and those who have contacted with them. Prolonged diarrhea was one of the major symptoms of giardiasis during an outbreak of this intestinal infection.

RESUMO

The paper deals with the topical problem of false parasitic diseases (FPD) in patients with various organic (somatic and mental) disorders. A classification of groups of FPD patients is given according to their clinical and social signs. Examples of 6 cases of FPD observed by the authors and those published in the literature are presented. Simple and effective recommendations on the management of FPD patients are given.

The formalin ester-enrichment test for giardiasis was used to examine 742 children aged under 14 years at children's clinical hospitals in the town of Perm in February to October 2000. All the children had symptoms of gastrointestinal lesions. Giardiasis was detected in 26.4% of the children; including 21% among 200 children aged 0-2 years, 26.6% among 297 children aged 3-6 years, 30.6% among 245 children aged 7-14 years. Various background abnormalities were revealed in 158 children, including 62 and 68 children aged 3-6 and 7-17 years, respectively. The background abnormality makes a clinical diagnosis of giardiasis difficult and lowers the efficiency of specific treatment. Macmiror and tiberal are the most effective in treating giardiasis in all age-group children. Tiberal is preferable for treating children without background abnormalities. Specific treatment for giardiasis should be combined with the use of probiotics and enzymes.

RESUMO

A hundred and fourteen cases of subcutaneous dirofilariasis caused by Dirofilaria repens were described in the CIS in 1915-1996. The mean age of the patients was 38.4 years (from 3 to 70 years). Dirofilaria was found in the eye in 45.9% of cases, in the head and neck in 17.5%, in the arms and hands in 11.7%, in the legs in 8.1%, in the viscera in 2 cases, and in the genitalia in 2 cases, they being prevalent in the body's right part. The north boundary of local infection cases was in latitude 53-54 degrees North. The maximum cases were detected in January and June. Dirofilariasis clinically appeared as a migrating subcutaneous tumor occasionally complicated by secondary abscess. The tumor generally contained an immature female, an immature male was found in one case. The sole treatment was surgical helminthic excision. There was a trend of subcutaneous dirofilariasis cases to increase in the RF in the past decades.
